Permission Forms
Our desire at TSM is to provide a safe place for your student to experience. Below you will find out 2008 Medical release Form. We ask that you would download this, fill it out, have us or someone else notarize the document, and turn it in to the Student Ministry Office so that we have a medical / permission form for the entire year.
We will ask you for permission for your kids to go on outings BUT we will have your medical info on file for the year. At each event we will carry this info with us in case our team needs to reach you in case of an emergency. Download it and get it to us ASAP so we have it for the year.
